We collect only the minimum information necessary to provide safe, compassionate, and effective psychiatric care.

We are legally required to maintain the privacy and security of your protected health information, to provide you with this Notice explaining our privacy practices, and to follow the terms of this Notice.
Information We Collect
We may collect personal information such as your name, contact details, medical and psychiatric history, treatment preferences, and insurance information. We may also collect limited non‑identifiable information, such as browser type or language preference, to improve website functionality and user experience.
How We Use & Share Your Information
Your information may be used and disclosed for treatment, payment, and healthcare operations, including providing psychiatric care, coordinating services, billing, quality improvement, and communicating with you. We do not use or disclose your information for purposes outside these activities without your authorization, unless required or permitted by law. Any use beyond these purposes requires your written consent.
Other Uses and Disclosures Permitted by Law
We may use or disclose your health information without your authorization for public health reporting, health oversight activities, judicial or administrative proceedings, law enforcement purposes, to avert a serious threat to health or safety, or as otherwise required by law.
Uses Requiring Authorization
Certain uses and disclosures, including marketing and the sale of health information, will be made only with your written authorization. You may revoke your authorization at any time in writing. Telehealth & Electronic Communication
Telehealth services involve the electronic transmission of information using secure audio and video technology. While Noema Health Nurse Practitioner in Psychiatry PLLC uses HIPAA‑compliant platforms and reasonable safeguards, no electronic communication is completely risk‑free. By participating in telehealth services, you acknowledge and accept these inherent risks. Telehealth may not be appropriate for all clinical situations, and in‑person care or a higher level of care may be recommended when clinically indicated.
Text, SMS, & Electronic Messaging
Text and SMS communication is used primarily for non‑clinical purposes such as appointment reminders, scheduling, and intake documentation. If you initiate a clinical request by text, we may respond; however, sensitive matters should be addressed through our secure telehealth platform or by phone. Text messaging is not fully secure. You may opt out of text or SMS messages at any time through Spruce or your patient portal, or by withdrawing consent in writing, except where we have already relied on it.
How We Protect Your Information
Your information is protected using administrative, technical, and physical safeguards, including encryption and access controls. Records are retained only for as long as required for care delivery or as mandated by law.
Documentation of Care
Telehealth encounters are documented in your medical record in accordance with the same legal, ethical, and professional standards as in‑person care

Your Rights
You have the right to access and obtain a copy of your records, request corrections to your information, request restrictions on certain uses or disclosures, and request confidential communications. You may also request that information not be shared with your health plan for
services you pay for in full out of pocket. While we are not required to agree to all requested restrictions, we will comply with restrictions related to services you pay for entirely out of pocket.
You have the right to receive a paper or electronic copy of this Notice at any time, even if you have agreed to receive it electronically. You may also request an accounting of certain disclosures made in the past six years, excluding those for treatment, payment, and healthcare operations. Finally, you have the right to file a complaint without fear of retaliation.
Withdrawal of Consent
You may withdraw your consent for telehealth services or electronic communication at any time by providing written notice. Withdrawal of consent will not affect care already provided or actions already taken in reliance on your consent.
Confidential Communications
You may request that we communicate with you in a specific way, such as at a different address or phone number. We will accommodate reasonable requests.
Complaints & Privacy Contact
If you have questions or wish to file a complaint regarding privacy practices, please contact our Privacy Officer at admin@noemapsychiatry.com. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. Filing a complaint will not affect your care.
Emergency & Crisis Limitations
Telehealth services are not intended for medical or psychiatric emergencies. If you experience suicidal or homicidal thoughts, active psychosis, or a mental health crisis requiring immediate intervention, you agree to seek in‑person emergency care. In situations involving imminent risk, Noema Health may contact emergency services or your designated emergency contact to ensure safety.
Emergency Resources
If you are in crisis, call 911 or go to the nearest emergency department. You may also call or text 988, dial 1‑800‑273‑TALK (8255), or text HOME to 741741 for immediate support.
Policy Updates
We may update this Notice periodically. Significant changes will be communicated via email, website posting, patient portal notification, or made available upon request.
